Membership Form - This form can be filled out and will use your default email program to send in an application. You can look in your sent folder to find a copy of what you have emailed to us (Subject line: "HHS Membership Form"). Print this "sent" email and along with your check for membership or renewal, mail it to the address listed in the "sent" email. This will help ensure your membership form and payment has been received by the library.
